The security incident, which occurred between June 24 and June 26, 2025, remained under forensic investigation for nearly a year. Wildwood Surgical Center confirmed on May 13, 2026, that the stolen data included protected health information and personal identification records. While the center secured its network and notified federal law enforcement immediately after the discovery of the breach, affected individuals are only now being identified for official notification.
Wildwood Surgical Center Reveals Massive Patient Data Breach
Patients at the Toledo-based Reynolds Road Surgical Center are bracing for the fallout of a significant data breach that exposed sensitive medical and financial records. Unauthorized intruders spent two days inside the facility’s network last summer, accessing files that contained Social Security numbers, diagnostic histories, and payment card details.

Management has advised those impacted to monitor their credit reports and financial statements for signs of fraud. The center is currently distributing notice letters to patients whose addresses are on file and has established a dedicated assistance line at 833-319-7579 to handle inquiries. Experts suggest that victims should also utilize resources from the Federal Trade Commission and major credit bureaus to initiate fraud alerts or credit freezes to mitigate potential long-term identity theft risks.
