Discharge planning, a notoriously complex bottleneck in acute care, is the target of a new partnership between UT Medical and TransformativeMed. The organizations are developing an agentic AI solution on the CORES platform, aiming to automate administrative workflows and alleviate the burden on clinical staff across the health system.
Dr. John "Clay" Callison, Jr., Chief Medical Information Officer at UT Medical, identified discharge planning as a critical starting point due to the high stakes involved in transitions of care. David Stone, CEO of TransformativeMed, emphasized that the development process relies on direct input from clinicians who navigate these workflows daily. This approach marks a shift toward operationalizing healthcare AI, moving beyond theoretical applications to address specific, resource-intensive hospital processes.
